Subject: EXPLANATION REGARDING SERIOUS IRREGULARITIES OBSERVED DURING VISIT
It has been reported that during a visit to Govt. Girls High School Chak No. 213 GB, Faisalabad on 24-08-2026 at about 08:30 HRS, the following serious irregularities were observed:

- The Teacher Attendance Register was provided approximately 30 minutes after my arrival, i.e. at 09:00 HRS. The Head Teacher arrived at the school through the emergency exit door, located behind the school office, and sat outside the office.
- The majority of the teaching staff, including the Head Teacher, were found late at the time of inspection.
- Poor cleanliness was observed throughout the school.
- The overall condition of the school indicated that it appeared not to have been opened on 20th and 21st August 2026, contrary to the directions issued by the Director Monitoring, PMIU.
- The name of a teacher was entered in the Attendance Register after my arrival at the school.
- The Head Teacher displayed highly rude and inappropriate behaviour during the inspection.
- The Head Teacher refused to disclose the names of staff members who arrived late.
- Only 02 out of 12 washrooms were found functional at the time of the visit.
- Electricity was not available in the school. The electricity meter had reportedly been removed by WAPDA officials due to non-payment of electricity bills for a long period.
- No drinking water was available in the school at the time of inspection.
- Heavy dust was found in all classrooms, including the Head Teacher’s office, indicating poor cleanliness and maintenance.
- The Roznamcha was not maintained up to the time of the visit.
- The attendance registers of all classes were not provided at the time of inspection.
- The school tablet was not provided for inspection.
- Mr. Akhtar Rasool (Naib Qasid) was marked on casual leave, whereas he has reportedly been on Ex-Pakistan leave since 01-06-2024, approximately for two years, without approval from the competent authorities. Furthermore, he was marked present in the Attendance Register from 01-05-2026 to 22-05-2026, while no attendance was recorded for him during June and July 2026. This matter requires verification and appropriate action.
The above observations indicate serious negligence, poor administrative control, misconduct, failure to maintain official records, attendance irregularities, and non-compliance with departmental instructions.
You are, therefore, hereby directed to appear in person before the undersigned on 31-08-2026 at 02:00 PM and furnish your explanation regarding each of the above-mentioned observations.
You are required to explain why, despite the above-mentioned serious irregularities and lapses, departmental proceedings should not be initiated against you under the relevant rules.
Your explanation should be clear, specific and supported by documentary evidence, wherever applicable. In case of failure to appear before the undersigned on the given date and time, or failure to furnish a satisfactory explanation, the matter shall be proceeded under the relevant rules, without any further notice.
